Hospital Accident Insurance

This is a complimentary program to our AD & D insurance.  Designed to help you defray the high cost of hospitalization as the result of an accident.  The plan provides cash benefits, paid directly to you, for each day you are hospitalized.  The first two months are free as a benefit of membership with our credit union.

The plan offers $200 a day, an amount that doubles if the hospital stay is in intensive care.  In addition, the plan offers payment for visits to an emergency room where insureds can receive up to $100 per visit resulting from a covered accident.  Family coverage is also available.

An annual mailing will be sent to primary members of our credit union with details and a special "benefit card".  Once coverage is effective, you can carry the "activated" benefit card in your wallet or keep with important papers.

There are no medical exams or lengthy health questionnaires to qualify for this insurance plan.  In order to activate this insurance coverage, simply complete the enrollment form included in the annual mailing and send it to the Plan Administrator before the deadline indicated.  Once enrolled, you will receive an Insurance Coverage Document.

The first two months of coverage are free, as a benefit of membership with our credit union!  After the first two months of coverage, premiums will be automatically deducted from your Fremont Federal Credit Union checking account or credit card.

This insurance is underwritten by Hartford Life and Accident Insurance Company.  They may reached at 1-800-860-7098 weekdays from 7:00 a.m. to 8:00 p.m. and Saturdays from 8:30 a.m. to 5:00 p.m. CST.  with any questions regarding this program, to cancel coverage or to file a claim.
